We are seeking a detail-oriented Nursing - Population Health Data Intern to join our Population Health Clinical team. In this role, you will help analyze patient data and clinical/medical information to create actionable insights for our healthcare partners, supporting their efforts to provide proactive, value-based care.

Key Responsibilities

• Review and analyze population health data using the Intus Care analytics platform to identify high-risk patients and trends.

• Review patient medical records to identify clinical trends and outcomes to inform population health initiatives.

• Create clinical reports summarizing individual patient profiles, population segments, and care strategies providing insights into clinical outcomes and potential strategies to drive improvements.

• Create and distribute weekly clinical intelligence reports highlighting patients with increasing risk scores, recent incidents, or concerning utilization patterns.

• Collaborate with clinical team members to understand care patterns and develop meaningful insights.

• Support healthcare partners in utilizing data analytics to improve care delivery and patient outcomes.

• Maintain strict confidentiality of protected health information (PHI).

Required Qualifications

• Currently pursuing a bachelor's or master’s degree in nursing field.

• Strong analytical and critical thinking skills.

• Excellent written and verbal communication abilities.

• Proficiency with data analysis and Microsoft Office Suite.

• Ability to maintain strict confidentiality and handle sensitive information appropriately.

Preferred Qualifications

• Experience with healthcare data analytics or population health management.

• Familiarity with value-based care models.

• Knowledge of HIPAA compliance requirements.

• Understanding of geriatric care and complex care management.
