Population Health Data Intern

We are seeking a detail-oriented Nursing - Population Health Data Intern to join our Population Health Clinical team. In this role, you will help analyze patient data and clinical/medical information to create actionable insights for our healthcare partners, supporting their efforts to provide proactive, value-based care.

Key Responsibilities
Review and analyze population health data using the Intus Care analytics platform to identify high-risk patients and trends.
Review patient medical records to identify clinical trends and outcomes to inform population health initiatives.
Create clinical reports summarizing individual patient profiles, population segments, and care strategies providing insights into clinical outcomes and potential strategies to drive improvements.
Create and distribute weekly clinical intelligence reports highlighting patients with increasing risk scores, recent incidents, or concerning utilization patterns.
Collaborate with clinical team members to understand care patterns and develop meaningful insights.
Support healthcare partners in utilizing data analytics to improve care delivery and patient outcomes.
Maintain strict confidentiality of protected health information (PHI).
Required Qualifications
Currently pursuing a bachelor's or master’s degree in nursing field.
Strong analytical and critical thinking skills.
Excellent written and verbal communication abilities.
Proficiency with data analysis and Microsoft Office Suite.
Ability to maintain strict confidentiality and handle sensitive information appropriately.
Preferred Qualifications
Experience with healthcare data analytics or population health management.
Familiarity with value-based care models.
Knowledge of HIPAA compliance requirements.
Understanding of geriatric care and complex care management.
